During internal submission, there was a typo in the filing name's last element, it had been entered as "d'Aqulia", not the intended "d'Aquila". That error was entirely mine and has been fixed in this External.
Several issues were raised in internal commentary by Lilie Ragged Staff.
First, per SENA Appendix A for Italian, triple given names are documented after 1460, so we need to document the whole thing to after 1460. What we have here is possibly a triple given name, an unmarked patronymic and a locative byname.
Second, the Wikipedia link for Guidobaldo does not meet our usual standards for documentation and Lilie RS was able to find the name only as a byname at Italian Names from 1364 and 1441: Name Elements, by Mari ingen Briain meic Donnchada - http://medievalscotland.org/kmo/Confraternities/NameEl... - the name element is listed under bynames as de Guidobaldo Largi.
The submitter has not ticked any of the Boxes of Doom(tm) so we appeal to the assembled heralds of the College for their greater Italian name knowledge.

Meadhbh is found in Mari ingen Briain meic Donnchada's "Index of Names in Irish Annals" (https://medievalscotland.org/kmo/AnnalsIndex/Feminine/M...) with Annals dates of 1444, 1502, 1555, 1577, 1582.
Tuathal is also found in Mari's "Index" in the same date range, with Annals dates of 1450, 1512, 1531, 1541 (http://medievalscotland.org/kmo/AnnalsIndex/Masculine/...)
In standardized post-1200 Gaelic, we'd expect to see this as Meadhbh inghean Tuathail, but spellings of ingen/inghean in the actual Annals are all over the map. The Annals of Ulster and the Annals of Loch Cé in particular continue to use ingen post-1200, as seen in the dated examples under Meadhbh.
Correction to Name (2023-Aug-02 22:08:42): I had intended to edit the spelling of this name to Meadhbh ingen Tuathail (lower case i on ingen) per Alys Ogres' comment on our Internal, but it slipped through.
Meadhbh ingen Tuathail is the version we intend to register. Apologies for the oversight.
