The translation of the parts in the red boxes on page 76 are:

Person number 5's name is then described:

"Asmenvardis, kuris nėra vardas, patronimas ar -sk- tipo antroponimas + patronimas. Asmuo užrašytas be prievardžių: Волкъ Яновичъ „Vilkas,(15) Jono sūnus"."

With the above information, we can then say that Vilkas should be used as a given name, based on it being highly likely it was used in pre-written Lithuania as noted in ffride's "A guide to some Lithuanian naming patterns." (https://heraldry.sca.org/names/slavic/ltpatterns.html#mas_pat.1)

Chizmadia - legal surname of submitter provided by driver's licence copy supplied with submission and allowed as per SENA PN1B2e

Chizmadia modern translation means "bootmaker" making it similar to an occupational by-name in period.

Name Order using ffride wlffsdotter's article, "A guide to some Lithuanian naming patterns", https://heraldry.sca.org/names/slavic/ltpatterns.html#characters shows pattern of [given name] + [by-name]. Occupational by-names follow this pattern as well. Therefore, Vilkas Chizmadia can be created.
