The formation given appears to be based on the following from Da'ud's article:

This is a kunya as the first element, not an ism. It seems "al-Yaman" is one of those names that's (as Da'ud puts it) both an ism and a laqab/nisba used as an ism. IOW, the name in the article is a kunya+an ism+a nasab+a "cognomen".

In short, the formation of this name has not been documented, and I find no documentation for it. I believe the name should be Safiyya bint Garsiyya al-Zarqa' al-Isbili
